Anna's Experts in Practice Journey
Hello! My name is Anna Yoxall – Adult social worker working in the Adults learning Disability Team since 2018.
I am part of the Experts in Practice project, my chosen subject area in Preparation for Adulthood. I am interested in this area from the perspective of supporting people transitioning from Children’s Social Care to Adult Social Care with a learning disability and or autism.
As a practitioner working with individuals and their families who are going through the transitioning process for preparation for adulthood. I often find that this is very complex as the planning is unique and person centred to each individual and there is no one box fits all approach.
Individuals and their family are not always clear on what the process is and find this a very daunting and stress invoking. This space is for individual’s and their families as well as other professionals to provide you with more information and resources around preparation for adulthood.
Support Available
The Adult Social Work Tool Kit can be used to support practitioners to support communication, understanding and interactions with individuals to support them to be actively involved throughout Preparation for Adulthood planning. It is hoped that with better communication leads to building more effective relationships and a greater understanding of what is important to the individual.
